I used to send a little card that said "Thank you for your purchase, your feedback on Etsy is greatly appreciated. If you have any concerns or questions about your order, feel free to contact me via my Etsy shop or at keisha.sibert@cupcakewishesandbirthdaydreams."

But I don't know if that was helpful, so I stopped doing it. Ironically, more people just voluntarily leave a note. I think now Etsy has a Feedback message that pops up at the top of the screen when you login and people might be adding it that way. Did I really answer your question? OOOOH - and you can always follow up with your customers a few days after you know they've received their orders. Just a little way to exceed their expectation - they might be prompted to leave feedback just from you reaching out to them.
